Emergency information

If you are an emergency case requiring immediate attention, please call 9-1-1.

Is my personal information safe?

Please ensure that your personal information is kept up to date with a current phone number, cell phone number, address, and emergency contact information. Please inform our receptionists of any changes to your personal information upon arrival at the clinic.

How do I get my test results?

All patients are expected to return for follow up on all test results with the ordering physician unless otherwise informed by the physician. We do not follow a “no news is good news” policy.

The staff is not authorized to give out test results over the phone.

What is your cancellation or no-show policy?

Missed appointments will be billed depending on type and duration of the appointment

Fees owed for a missed appointments must be paid by patient before they can be seen by any physician at our office.

If you have a special circumstance regarding a missed appointment, you may discuss this with our staff.  The staff are not authorized to make any decisions regarding office but will consult with the physician.

Doctor-Patient relationship

In order to provide the best possible health care for the patient, a satisfactory doctor-patient relationship needs to occur. If for any reason, this relationship becomes compromised it may be terminated. If this occurs, we will provide emergency medical care only for one month, or when the patient finds a new family doctor, whichever comes first.
